July 30, 2016, Sports Physicals and Cardiac Screening at Willow Springs
For your convenience, the Lovejoy Sports Medicine Department is offering sports physicals and hosting Who We Play For for cardiac screening at Willow Springs on Saturday, July 30th. Physicals and cardiac screens are open to all 2016-17 Lovejoy 7th through 12th grade student-athletes. A parent or guardian should attend with the student to sign permission forms. 2016-17 REQUIRED FORMS FOR ATHLETICS
1) 2016-17 LISD-issued Student Athlete Information Packet
Packets are required and must be completed before participation in any tryout, practice or competition. The packet has 12 pages and is printed on 11×17 light purple paper, folded & stapled. Athletic Information Packets are NOT available online. Packets are available for pick-up only from the LISD Athletic office, District office, LHS office or WSMS office. For 24 hour access, there is a box of packets located in front of the small field house behind LHS.

Where do I turn in the completed packet?
You may turn in completed packets at the Athletic Office, LHS Office, WSMS Office or SCIS Office during office hours.
2) ImPACT Test
All Lovejoy athletes will need to complete the ImPACT test before participation in practices or games. The ImPACT test provides a base-line computerized neurocognitive assessment that is used by medical doctors, psychologists, athletic trainers, and other licensed healthcare professionals to assist them in determining an athlete’s ability to return to play after suffering a concussion. This test takes about 45 minutes. ImPACT testing will be scheduled by each coach for their team members. The ImPACT test is good for 2 years.
3) Emergency Card
The Lovejoy ISD Emergency Procedures Release, Oral Medication Release, and Sports Medicine Department Policy & Procedures acknowledgement is printed on a half sheet of white card stock. Coaches distribute this card at pre-season meetings and carry emergency cards to games and practices. One card required for each sport.
New Student Enrollments – PAPF
If you are a new student enrolling in Lovejoy ISD and you want to participate in high school athletics, you will need to fill out a UIL Previous Athletic Participation Form (PAPF). Please visit the UIL website Forms page to print a PAPF form: http://www.uiltexas.org/athletics/forms
Team and Individual Photos
Purchasing photos is optional. To purchase photos please complete the Buzz Photo order form and give the form with payment to the photographer at the time of your team’s photo shoot.

Alternate Transportation to/from a Competition
It is the policy of the Lovejoy Athletic Department for each athlete to travel to and from the site(s) of all athletic events with the team. Except in an emergency situation, an athlete must have prior approval via the completion, submission and approval of this form for all forms of alternate travel. Parents or designee must personally sign for the release of his/her son/daughter before an athlete may leave any athletic event. Forms must be submitted to coach no later than 24 hours before the athletic event.
